The goal of this project is to develop an AI-powered CareHub intake agent to transform how patient transfers are managed across its network. By integrating real-time data on hospital capabilities, physician coverage and patient context, the agent will provide intelligent, ranked recommendations to optimize care routing and system capacity. The goal is for the agent to automate up to 60% of straightforward transfer decisions, allowing human teams to focus on the most complex cases with AI support as a copilot.
